Reporting Prohibited Conduct

Use this form to report violations of the Illinois Gambling Act, Video Gaming Act, or Sports Wagering Act, or to report prohibited conduct related to the outcome of sporting events. Reports may also be submitted through the Illinois Gaming Board Hotline at (855) 494-0237.

The identity of any individual making a report and the contents of any report are confidential and not subject to disclosure under the Freedom of Information Act. The identity of any individual making a report shall not be disclosed for any reason unless the individual authorizes that disclosure or if the allegation is referred by the Board to a law enforcement agency.
